  2. Yes and not enough, in that order.  ;-)

    Salaries are more or less set across the board for any particular hospital for all residents (interns are first-year residents).  It varies a little bit by hospital and by what area of the country you are in (there are slightly higher salaries set for residencies in major cities where the cost of living is higher).  Some hospitals' residents have collective bargaining agreements, and there is a little wiggle room for institutions to offer perks like meal and book allowances, and there are sometimes opportunities for moonlighting shifts and extra pay for chief residents, but salaries can't be negotiated individually.  

    Salaries start around the low $40K mark and increase in every year of residency, but you have to look at the individual hospital to know exactly.
