  3. No, they do not put worms in margaritas at bars. You're thinking of tequila, which does not contain a worm either.

Mezcal, an alcoholic beverage similar to, and often classified as, tequila does contain worms. However, this is not the kind of worm you're thinking of, like an earthworm - it's actually a special type of worm that is quite good.

It is added to the bottle during the packaging so as to impart a particular flavor on to liquor. The flavors can vary according to worm, but usually add a sweet and slightly woody flavor. The worm is quite crunchy, creamy, and sweet by itself.

When you do encounter a bottle of mezcal you can choose to either eat the worm or not. Actually, very few are brave enough to try the worm, but the ones who do will rarely throw the worm away in future bottles.

Try it one day, both mezcal and the worm, you might like it enough to keep it a permanent addition to your liquor cabinet. :)

By the way, it's virtually impossible to find mezcal in areas like mine. You will have to order it.

  4. Not unless you request it!

Really, the worm is just for show in the bottle.  Bartenders also use pour spouts that the worm wouldn't be able to fit through, anyway.

What prankster friend of yours fed you that lie?!  Maybe you can request for the bartender to put the worm at the bottom of your friend's drink so they get a fun surprise when it's gone.  ;)
