No, you don't need to have a picture of them, but it really does help...You need to have either a mental image of the person, or you can use something that belongs to them, in order to properly target the spell.

Not a good idea to try to cast a spell on another person, especially someone you do not know.  Spellwork should only be on yourself and only for your good or the good of others.  You could send out a healing spell to someone you knew, but you have to have their consent first.  Trying to cast a spell on someone without their okay invites having the spell rebound on you 3 fold.  If it was an ill-intended spell, that would be really bad, wouldn't you agree?  Don't do it!  For your own sake!
