Do u take ur child to an in- home daycare?

If so, what papers did u and the child care provider fill out?

  1. As a parent you should be required to fill out (or have doctor fill out) a health form and/or shot record, you should sign a page indicating that you have read the parent handbook, you should complete an emergency card/form and list people who are allowed to pick-up your child, and paperwork containing basic information about your child's likes, dislikes, allergies, developmental history and special needs.  Some providers will ask for your driver's lic. # in case you give them a bad check.

  3. i did, and she pretty much had the same forms you would see at any daycare.... consent forms, liability/ insurance forms, eating habit questionaires.  i dont remember what else, but my daughter's child care provider is licensed through the state therefore she has to meet similar requirements to other day cares.
