  1. Yes, because (Old) Lyme is the name of a place in the USA from which the disease was named.


  2. You capitalize "Lyme" because it is a proper noun--the name of the town in Connecticut where an outbreak occurred.  You do not capitalize disease.

  4. Sounds strange but you only capitalize the word Lyme and not disease when it's in the middle of a sentence or at anytime for that matter.

  7. You always do... If you specify a certain disease's name then it's capitalized. You wouldn't, however, capitalize just 'disease'.
