    associates degree -> bachelors -> masters -> Doctorate

    So no.

    However, one may be able to apply work that is done in pursuit of a masters to a doctorate. It would all depend on the particular school policy.

  4. Some universities in the UK allow you to do a joint Masters and phd....all in 4 years (intensive). I think it also depends on where you apply and the amount of subject knowledge you already have. Check around you may luck out on your own personal experience.

  5. It depends on the course of study, and the school. I'm going to get a PhD in psychology at either UC Davis or UC Berkely, and neither school requires you to get a masters before a doctorate for my course of study. I've talked to counselors at both schools and have they have told me that a masters is not necessary for a doctorate, although it is recomended.

No, depending on the University, you can surely start as a P.H.D candidate in a graduate school, the accrediation of the P.H.D. program of choice is the most important when knowing you will complete it.  Other considerations are time and money and the possibility of bowing out of the program of choice with some type of diploma.
