It is a reference to the "Ghosts" that are supposed to haunt so much of Nashville.  In this case we are talking about the ghost of Hank Williams SR.  The Cadillac is the hint that leads to Hank.  He was an alcoholic and died in the back seat of that Cadillac while on his way to a show.
