The earliest one I have ever seen an account of is the Bath School disaster, in Michigan in 1927. A school board member (and local farmer), angered by rising property taxes to fund the consolidated school, killed 43 people before taking his own life.

It was pre-meditated. His wife was the first victim, school children and teachers were killed by explosives placed throughout the school, then the Superintendent and the killer himself died as the result of a car bomb.
