Technically all travel into the US except from a US territory requires a valid passport.  Since you are travelling to Canada in theory you could fly without a passport however you would not be able to return to the US by air.  In practise airlines can require you to present a passport for travel outside of the US and can refuse you boarding if you fail to present one.  If you are a citizen of Canada returning home then boarding privileges can be granted however if you are a US citizen then you may find yourself unable to board without a passport.

  3. yes because you are flying out of the country.

    only domestic flights (within the US including hawaii and alaska) do not require passports except for just an ID to confirm your ticket.  

  4. Yes.  As of this year, a passport is required for US citizens.  You used to be able to show a birth certificate or other identification.
