Depends on how you get there. Although, if you take a cruise ship you have to have a passport to board and to get through customs at the end of the trip. During the trip, you will not need it to take excursions etc. If flying in, definately. Not sure about driving there, but I believe you will need one at the border.

Yep - you need a passport to go anywhere outside of the US, except for US territories like the Virgin Islands.

You used to be able to go to Canada and Mexico with just a birth certificate, but due to new security measures, you need a passport.

You MUST have a passport to FLY anywhere outside the U.S. or its territories.  You do not yet need a passport to travel by LAND or SEA to Mexico, Canada, or the Caribbean area.  See site below and read top half of the page carefully for the current regulations.  See 2nd site for all passport application info.
