  1. yah, you probably should

  2. I would include book and author.

  3. If the quote is extremely well-known, so that any educated person knows the source, then you don't need to cite the book; otherwise, you do.

Find some books by Winston Churchill in your library.  He wrote a great deal about World War II, he was one of the three or four most important figures in the war, and when you find something worth quoting, the book will be there in front of you.

Be aware that there was an author of fiction named Winston Churchill.  If you find one of his books, put it back on the shelf.

  4. I'm not your teacher, but I can tell you if I didn't have the author, book title and page number of the quote, I would be sadly disappointed and certainly reduce your points.  Sprucing your report up with as many seemingly useless figures and charts as possible really makes it look well-researched and adds pages of official-lookin' bulk. Play that little game to the hilt---you'll be surprised.  It really makes a difference!
