  1. Depending on where you go, "service" may be included.  Most places I visit do not do so - a tip left on the table or given to the waitress when paying the bill is welcomed.

    normally you do but there is no fixed percentage like e.g. in canada - i think 5 - 10% is appropriate (that's what i tip) but it depends on what you're consuming. if you pay per credit card there is often a line where you can fill in the appropriate amount but it is better to tip the waiters cash as they sometimes don't get the amount you remitted for them.

  3. "Service" is almost always included in the bill at restaurants in western Europe. The local practice is to leave the coin change on the table (when paying with cash) in addition to what is included in the bill.  However, I always leave about 5% tip which seems both appropriate and welcome.
