  1. I believe you do, if the sentence uses the word "book."

    I was reading the book, The Monsters on Elm Street, (title would be underlined) when there came a knock on the door.


  2. You should treat a book title like any other noun. Sometimes commas are needed, but often they aren't. it depends on the context. For example:

    This semester we'll be reading TO KILL A MOCKINGBIRD in my English class.

    No need for commas there. But in this example, commas are necessary:

    The book I read last semester, TO KILL A MOCKINGBIRD, was written by Harper Lee.
