Documents to take to pediatrician?

I recently moved from New Mexico to Arizona and i need to take my daughter to a new pediatrician. I don't have the old doctors records yet. what documents do you normally have to bring for the first visit?

  1. Call your doc. and find out. Normally her old records will be sent to her new doc with your request.  


  2. They will have you sign of form that releases info from the first doc, to the second.  Try to sign that form and get it faxed to the old doc before your first visit, that way your new doc will have received all the info from New Mexico when you come to your first appointment.
