  1. It's either the wrong belt, installed the wrong side facing in, or one of the pulleys is out of alignment. The tension could be off as well. Even with a tensioner, you still need a gauge. The spec should be on the belt path decal under the hood.

  2. something isnt aligned properly if you are volvo techs you should know that, take it to a dodge dealership and have one of their master techs check it out if you cant find the problem

  3. have you checked all the other pulleys on it,because any one pulley that the belt runs on will cause this to happen, even one of the pulleys having a slight bend in it will cause this, and also anything that's been changed on it like power steering pump o the alternator that the pulley didn't go all the way back up on will cause it,i got in to one like that, and it was really hard to find, but when i got back and looked at the angle the belt was running on i found that the bolts were loose in the power steering pump causing this too happen,go back over it and check everything and see if te belt is running on an angle instead of running flat, good luck with it.

  4. i've had that exact problem on the exact vehicle.

the problem is this.  a sloppy design.  so much so that some of the vans of those years were recalled or have a TSB for them.  take it to a dealer, there is a specific spacer designed just to fix that problem, and if the mileage is low enough on the van, they may fix it for free because of the history.  mine was partially covered.
