4d scans are not medically required.only 3d. so no they don't but you can ask your doctor for one, it will cost about a hundred bucks. more or less depending on the place. i have insurance through the state that is how i know this

because the 2D/3D scans that the hospital do are sufficient enough to tell you if something is wrong.

Insurance companies kinda see these 4D scans as ''cosmetic'' and have nothing to do with health. Its just so you can have a few lovely pics and a dvd.

Insurance covers ultrasounds for medical purposes only. If they are doing something medical in a doctor's office, they might cover it. Most 4D ultrasounds are done in ultrasound clinics for non-medical purposes, so most of the time they are not covered.
