  3. The first question that is going to be asked is: Did you file an incident report?  Unless there is something on record with your employer as to the nature of the accident, when it occurred, and how it occurred, then in the eyes of worker's comp...it didn't happen.

  4. Hi there. Were you released from employment because of the hernia? Can you prove that it was due to your job? Do you have a history of Doctor visits or can you get your doctor to verify the possible causes of the hernia to be work related?
