You apply to medical schools through AMCAS and yes, all schools are informed of your selections.  Many schools, if you are invited to do their secondary applications also ask you to list the other schools.  But this isn't anything to be concerned about.  No school expects you to apply to just their school.  The average number of medical schools applied to is 13 (based on AMCAS statistics).
