  1. Here is another site for lawyer referrals

  2. How long have you been disabled for?  It seems like it's been a long time if your long term disability has run out.  You need to get all the paperwork for the long term disability insurance carrier to see if there is anything to get that extended.  Contact your former employer if you don't have everything that explains it.  But, it doesn't last forever.

Appeal the decision by social security.  You should be able to find an attorney who specializes in social security appeals in the telephone book or get a referral through the local county bar association.

  3. One part of your question is for a lawyer referral. One site is http://www.findlaw.com/ or there are other sites.

Sometimes, ss disability will reverse itself on appeal. If you are provably disabled, this is likely. You could appeal alone or with a lawyer.

If you can extend your long term disability with your employer, your HR department would be the place to start. Don't hold out too much hope for that.
