Haha, I researched this out of curiosity for myself. It is actually the forehand grip Andy Roddick uses, with the index knuckle on the 4th bevel n the heel pad also on the 4th bevel. It's an unorthodox grip for a flat forehand like Gulbis', but it works for him, so no worries. Hope this helps.
