  1. Call the courthouse where you live and find out which court handles evictions (if court action becomes a necessity), then see the clerk of that court and ask for an example. Because landlord-tenant laws vary from state to state, you have to make sure that your eviction notice states anything in particular that the law requires it to say.

Where I live, the court that handles this has a blank form they give to anyone who asks - so you don't have to re-do the notice to comply with the law before you sue (if you're forced to).

  2. Try http://docstoc.com

  3. Dear :

I wish you and any guests or fellow occupants to leave the above referenced premises now in your possession by [3 business days from now]

**You are being asked to leave the premises. If you do not

leave, an eviction action may be initiated against you. If

you are in doubt regarding your legal rights and

obligations as a tenant, it is recommended that you seek

    legal assistance.**

    Regards,

    Landlord

That's all you need. The part between the Astrix's is required by the ORC 1923, which governs evictions in Ohio. Don't change or alter that part, it is required word for word.

I would further research the Revised Code and make sure you do everything by the numbers as well, or talk to a qualified attorney.

  4. You cannot serve an eviction notice - that is done through the courts, but you can deliver a notice of termination.
