Does anybody know where I can get a chart or a graph that can track a secondary students disclipine?

The best ones are the ones you make yourself because you know what kinds of categories you want to put on them, AND you can make them really cool, especially if you teach middle or high school.  Go to your local movie rental place and ask for some old movie posters (they'll give them to you for free) - get the ones that are a bit aggressive - you know - the one that have guys/girls with weapons on the front.  Cut out the movie stuff so you just have the picture and then put it in the upper left hand corner behind your chart with a cool caption like "You did WHAT today???  I don't THINK so!"  and then in the lower right hand corner for good behavior, get a poster that has more of a "feel good" picture with a different title like "Now THAT'S more like it!  At last, the perfect student!"

Have fun with it - let the kids help come up with the titles and help you make the poster; let them help make the categories for the chart too.  The more input that kids have, the more responsibility they'll take.
