Does anyone have a link to an informal preschool observation form for skills?

I need something informal to use with 2-4 year olds to monitor skills acquired/lacking. Basically a checklist. Thanks!

    do you take anecdotal notes? if you're using something informal, such as a checklist, i would have notes to back up the checklist information to share with parents. just make sure that the anecdotal notes include enough detail as to the situation, not just 'ann spoke in a compound sentence'. rather, 'when ann was playing with joe in the art area, she turned to joe and said, 'i need that red and blue one.' then, she pointed to the red and blue tissue paper.

    that type of note shows the interaction with another child and understanding of colors as well as the compound sentence.
