I know of 2 that participate with the company that I am with. It costs 19.95/month for the plan for the entire household. it is a discount plan and you get a 20% discount for ortho. The exact price depends on their normal fee, The plan also comes with  vision, chiropractic and prescription discounts. Email me if you'd like more info.
