Does anyone know a site where i can find 2 record-keeping forms found at early childhood centers?

i need a sample of an accidental report and an emergency form. its for my CDA class.

I would suggest asking at a preschool or day care center.  They should be very willing to give you a sample, blank form.

I'm not sure what you need for your emergency form--but here are a few sites that I have found that offer a variety of forms that can be downloaded.  

http://www.ccvillage.com/index.html -- This site has a variety of printable business forms--however you have to pay to download them.

http://www.childcarelounge.com/printform...  --This site has an accident form that you can download for free.

http://www.geocities.com/dormanfcc/Forms... Free forms that you can download and print off. This site has an incident report--which may work for your emergency form. They also have a medication form as well. I'm not sure what you need for you emergency form.
