Does anyone know about a nuclear accident in Piqua Ohio in 1960, give or take?

Piqua Ohio at one time had a small nuclear plant. It was shut down in, I think 1960, and dismantled. I have recently heard that it closed due to an accident of some sort. Anyone know anything about that?

The plant operated as a demonstration project between 1963 and 1966. There is no evidence of any contamination.
