  1. Paul and Maison Blanc both in Richmond, and in Chiswick Mortimer & Bennett Turnham Green Terrace and a deli across the road and a few yards just south of the tube station:

    Maison Blanc 27b The Quadrant, Richmond tel 020/8332 7041, Richmond tube; branches all over London. French patisserie next to the tube where you can pick up a picnic of bread and pastries. Mon-Sat 8am-7pm, Sun 8.30am-6pm.

This is a very good question.  I wasn't sure whether you were interested in buying bread in West London (you mentioned Chiswick/Richmond) or were specifcally trying to find the original source of where it was baked.  In Chiswick you can get Sally Clarke's (based in Kensington) bread from Mortimor and Bennet, the deli in Turnham Green terrace. There is also Maison Blanc opposite, who do make bread but not on these premises.  In addition there is a Polish shop just by Turnham Green tube which sells dark rye bread, and there is a street stand on Saturdays just outside M&S selling organic bread, though it does not look inspiring.  

    The dark horse is the new range of gourmet bread you can get at the Sainsbury's local in Chiswick.  The olive bread in particular is very good indeed. At £2.99 a loaf it is as expensive as Clarke's bread, but I actually think it is better.

    However if you question was really about finding a baker who makes bread n the premises then I don't know of one in Chiswcik; I'm not sure about Richmond.  Sorry I can;t be of more help.
