Cafe Edelstein (has pretty men dressed as school boys) - Short walk from JR Harajuku Station, and both the Tokyo Metro Meiji-jingu station and Omotesando station.

Those are some good ones. Most of the websites have maps, if not just ask for directions with the address. There's often a police box (koban) near many stations.
