Does anyone know exactly why this happened?

why was the 17 yr old boy(with cancer) in California at first denied treatment coverage by Pacificare.

why did they do that?

It depends on the kind of treatment he was receiving. Many insurance companies wont cover treatment if it is considered "experimental" or if it hasn't shown progressive results. Its really unfortunate so many people in a situation like that can not be properly treated b/c of the insurance companies.

There is a sneaky slithering snake in the US we call big insurance and its eating Americans alive.  By denying someone who is an insured, means keeping the money in the fat cat's coat.  See Michael Moore's "Sicko" documentary because it sends an alarming message that Americans are being fooled.

I don't even know who you're talking about, but you're talking about PRIVATE MEDICAL RECORDS.  

I'd imagine if he had other things wrong with him - like end stage renal failure - they wouldn't do chemo, because it wouldn't prolong his life.
