I guess that you are asking about:

"Cartilla Nacional de Salud".

It is an official document (the "Secretaría de Salud" gives them) to keep the record about vaccinations and other health issues. The most important is "Cartilla de Vacunación", wich is given to avery child, then now exists the new"cartillas":"Cartilla de Salud de la Mujer" and "Cartilla de Salud de los Adultos".

The document must be kept by the owner, if you don't have it it means it is lost. If you know who was the doctor (if private health care) or  in what hospital or house of health you got the attention, maybe they can have some records.

Injection Records? I think that you would have to go there in person, then show ID at the office where they were taken. If not call Mexicos Secretaria de Salud and aks if they can ship them to you
