  1. You'd be better off buying a book on Patios at your local building supply store, because there are many details to cover.

The basic process involves levelling, compacting, creating a base of gravel or sand, and more compacting.   Then, building forms, installing re-bar, creating expansion joints (for large areas), pouring concrete, levelling, finishing, setting, removing forms and curing (i.e. preveting from drying out before crystalization is completed).

It's a lot of work, and when it comes to the pour, you may want a helper if the area is more than about 8 feet wide.   Don't expect the concrete truck driver to get involved beyond putting the concrete where you want it.

  2. List below is where I found the best resources for your project.  Also remember to get an accurate measure of cement, try this formula:

measure the length x width.  Your will probably use 4 in of gravel = .33.  So lenght x width x .33.  Take this measurement and / by 27.  There are 27 cubic feet in a cubic yard, so just divide by 27 to get the correct amount of yards of cement needed for the job.

  3. I hate to say this, but if you are asking questions such as these, you may want to consider a contractor.  They can pour the different levels that you want, get the job done quickly, and be done.  They will also make sure that the substrate is properly compacted and level, which will make your pour last for years.  There are alos different types of concrete available, and a contractor would know which mix is the best for your area.  Hope this helps.
