Does anyone know how to fill in application form 6.4 to set aside a statutory demand?

I need to fill in this form and the 6.5 forms, does anyone have any idea how this can be filled and would you be able to perhaps send me a sample of how it should be filled in.

In any case, if in UK, take it to your local or any Citizens Advice Bereaux

You should have a leaflet with it explaining it. However I would take it the Court office or CAB  for help. but do not ignore it. In any event you are going to need proper advice particularly if you are a Limited Company
