  1. 3 wires.1 is black sometimes blue or red & hot,one is white& is neutral, one is green or bare & is ground. You can use electrical tape in a pinch but wire nuts  preferred. If you have a place where they connect inside the fan,if you get the neutral and the hot wrong ,it wont matter just make sure you get the ground connected correctly.Good luck.

  2. There should be some kind of strain relief where the cord goes into the fan.  If you can get that out you can probably just use the existing wire (so it fits through that strain relief), shorten it a bit and hook it up inside the fan.  Another way is to remove the strain relief, put a rubber grommet in the hole, buy new cord or use the existing  cord.  Once you have it routed inside the fan, put a cable tie or two on the cord to keep it from pulling out.

  3. If the wire got ripped out of the actual motor itself, then you may as well just scrap it and go get another one, it would be way more trouble than it's worth to try to get the cord back into the motor, or the switch for that matter.  If, however, it's pulled loose at or just behind the strain relief, then Wired's advice is quite good.  Don't pay any attention to Alwayson as he's assuming that box fans have a three wire grounded cord, which they haven't for over twenty years.
