Economic Stumulus Rebates are sent automatically, after you file your 2007 taxes.

If you normally do not file taxes, you will need to file this year (before October 15th, 2008) in order to receive the rebate.

Stimulus checks are automatically sent to you via the same way you received your tax return. If it was direct deposit or mailed you will also receive your stimulus the same way. You can go to the irs web sit and answer a few easy questions based on your return and your ssn# and the will calculate the amount and give you the date your check will be sent.

Are you talking about the 1040A tax return form for something like social security income?  See http://www.irs.gov/newsroom/article/0,,i... and click on the link to package 1040A-3 if you didn't get a copy in the mail.
