Does anyone know what sliding scale income guidelines are?

I don't want to bother with registering at a certain dentists office if I make too much money. What is the sliding scale chart? How does it determine how much I would have to pay with an income of $4500? The dentist office won't tell me, but I know there has to be a chart somewhere that they go by, what is it?

Unfortunately, there is no general chart. Each office sets up it's own guidelines.

Seems odd they won't disclose that info, though...
