Does anyone know what the names of these Star Trek episodes are?

1) The episode of voyager where that crew member that died comes back to voyager after some alien race brought her back to life and changed her

2) The episode of DS9 where Nog loses his leg

3) The episode of Voyager where B'elana gets all mad about the Maquis getting killed and turns the safety protocols in the holodeck off to try and hurt herself

4) The episode of Voyager where Tom tries to help a species fix their ocean, against Janeway's orders, and gets demoted to ensign.

1) There's an episode called Fury, where Kes comes back to Voyager, but I don't know if that's the one you mean.  It aired 5/3/2000.

2) It's Only a Paper Moon, aired 12/30/98  (A really good episode!)

3) Extreme Risk, aired 10/28/98.

4) Thirty Days, aired 12/9/98.
