  1. To add to what others have said, if your birth certificate has the names of both your parents, the letter needs to be signed by both of them.  If there has been a divorce then you need to show a copy of the custody order that grants custody to one parent if you can get only one parent's signature.  It would be a good idea if your parents are available by phone when you are crossing the border in case the officials want to check that everything is ok.  And you will need to be in the company of someone who is 18 or older, and the letter from your parents must give permission for you to cross the border with that person.  For identification you will need your birth certificate and a government issued photo ID such as a driver's license.  Best of luck with your plans.

  2. If you are flying, you'll need a passport.  If you are driving/crossing by land, you'll need photo ID and proof of citizenship (either passport or birth certificate).  You should have an affidavit signed by your parents, authorizing someone in your party (who is over 18) to be your "guardian" while you are in Canada.

  3. Under 16 you tend to need a signed letter from your parents.  It doesn't hurt if you have one at 17 though.

  4. If you're flying - passport. If driving, walking, cycling or coming by boat - government issued photo ID and a birth certificate or green card

  5. A passport.
