usually you can get them at restaurant supplies stores, they have every shape and size.  Usually they are pretty cheap there as well.  You can also try ikea in their kitchen section or even Williams sonoma or Crate and barrel.

Getting the exact one made by the World Tableware Div of Libbey Glass - The pattern is called Slate. See if there is a Restaurant Depot in your area. You need to be in the business so if your not you gotta no somebody who is in the biz orr see if your Manager of your fav restaurant will help
