yes. usually a $1.50 fee from your bank and a fee from that ATm itself. can be up to $5.00 total from some machines

Usually.  You may be special and your bank pays the fee, but I doubt it.

Yes, but It depends on the bank and it doesn't matter if you have Visa or Mastercard debit card.  There is usually an ATM fee and then there's another fee that your bank charges, so be careful taking money out from a different ATM other than from your bank.
