  4. No. The ball is not knocked on if a player heads/is hit in the head by the ball and it goes forward. Only a forward projection of the ball from the arm/hand of a player that hits the ground results in a knock on.

  5. By IRB Law 12 it states that:

    "A knock on occurs when a player loses possession of the ball and it goes forward, or when a player hits the ball forward with the hand or arm, or when the ball hits the hand or arm and goes forward, and the ball touches the ground or another player before the original player can catch it."

    So the answer to your question is that NO it isn't a knock-on if the ball comes off your head.
