Does olive oil need to be refrigerated after opening the bottle?

I never have, it doesn't to refrigerate either so that tells me it's safe not to !

NO!  The enemies of olive oil are heat, light, and air.  Just keep it in a cupboard and use it within a year of the harvest date.

Mine is in a dark bottle on the counter.

never keep oil in the refridgerator... it will solidify. it should be kept at room temperature.

no! It kinda separates and look gross! You can still use it though, It's just not very attractive.
