so i just got a vaccine for school, and well the nurse gave it to me, and i didnt get any documentations or anything back. im guessing that the doctor's office will send it to my school automatically themselves, because they did ask me which school i went to. should i have my parents get a copy of the documents and have them send it to the school or something?? please help

  1. Don't count on the doctor's office (or anyone) doing anything automatically.  You should  get a form from the school, take it to the doctor's office, and have them complete it.  You may have to drop off the form and pick it up later.  Once completed, you take it to the school and turn it in.  This way, you know things have been done that you need done.

  2. No it is your responsibility to get your records and give them to your school.  In most public schools in the US, you technically are not allowed to take classes until they have the records.

  3. Well I can say from just a recent experience that they dont.  We moved to a different state and my children's school records never got transferred, so I had to chase down everything in a different state myself (including the shot records)

  4. Schools are often pretty slow with that. You're doctor won't send it to the school. You'll have to bring your immunization records into your school office, but if I were you I'd just wait until the school asks for them and save yourself the trouble. I mean, they might never go looking for it, and then it was a waste of time. If you need your records, just call your doctor's office and ask them how you can get your immunization records because you need them for school. They'll know what to do.
