  1. If you subcribe to the service no - you only pay the flat monthly rate. Some companies allow you to use it on a per use basis and in that case you get charged each time you use it. If thats a feature you use a lot you might consider subcribing to it and only pay the flat monthly rate instead of on a per use basis.


  2. Typically, 3-way calling cost extra unless you have a "premium" service package that includes 3-way calling (check the calling features list of your phone package).  It is not usually a "standard" feature.

    Many VoIP services allow 3-way calling "between account holders" at no extra cost.  For example, if you and 3 or 4 of your buddies all have Skype, then you can 3-way or 4-way call with them on their Skype names at no charge.  

    But, if you want to 3-way to regular PSTN phone lines from Skype, then you are charged for the minutes of each person on the line who is not Skype account holder.  But, if you have Skype "Unlimited" CallOut service, 3-way calls to USA/Canada PSTN phone lines are no extra charge.

    The same holds true for most VoIP services.  Between account holders free - otherwise per-minute charges apply (some exceptions for unlimited packages - like Skype, etc).
