Don't know Passport Number for Mother's Passport?

When I came into the US at the end of 92 I was just a few months shy of turning 3 and was on my mother's passport. Now in order to leave the country I need to get my own passport. And she has apparently thrown hers away years ago. The questions I ask are, on the LS01 is it 100% necessary she fills out a police report since she destroyed it herself? And where can we find the information such as exact date of issue, and the passport number? Or any other random details the LS01 form might want that I might not be able to give them?

  1. This may not help, but when I filled in my passport form the parents passport numbers were optional though useful to confirm your identity. You could phone the hotline and double check if they're absolutely needed.

    To find the information the only way is usually to get it from the passport itself. Obviously since it's been destroyed you can't, for security reasons someone can't just go up a desk and ask for information on someones passport details.

    Try contact the embassy and ask what you can do.
