If the scene about the heart murmer was the foreshadow of things to come and Lizzie dies in childbirth Jill will move to live with Nigel to look after the twins and Phil too!  Lots of scope for tension between grannies too.  Or Julia will leave.  The kids need to be with dad and Nigel has to stay where he is to run the new ventures.
