Employee performance evaluation for restaurants and other hospitaly businesses?

i have a homework in management about evaluating employees. i wanna know how often is this done and what benefit can the employee get? and also,can anyone please share a copy of their restaurant's employee appraisal form

I haven't worked in this field in many years.  When I did, this was done yearly and it coincided with yearly raises (though other raises were given mid-year).  It was supposed to be to help the employee, but it just rehashed what the employee knew the boss already thought about them (you saw the boss many times during the day and he wasn't exactly quiet about how he thought you were doing).  Nothing to share, its been so long.
