Employment Application?

On my application it says

Name and Adress of Employer

Name of Supervisor

I worked at an amusement park, and from what i know, only my manager ran everything in the unit. i don't think we did have a supervisor that watched our manager or anything, but even if i did, i wouldn't know their address or their name.

Should i leave the Name and Address of Employer part blank and just fill in the Name of Supervisor instead?

You are misunderstanding the application:

Name and address of employer: That's the name of the amusement park and the amusement park's address.

Name of supervisor: That's your manager who ran everything in the unit
