I don't think that you should read directly from paper as you will need to maintain a good level of eye contact.  But some notes and questions that yuo have prepared looks like you have taken the time to consider what you want to say and the interviewer should see this as a positive.  It's worth considering that the interview could go anyway, and may sway from some answers that you have prepared, so don't plan responses too hard as it may throw you off course in the interview.  Keep notes clear and simple.

    What kind of job are you going for?  It might be worth buying or borrowing a smart shirt for the interview, just to give the right professional impression.

  4. No it is not a good idea. It would demonstrate that you have poor retention, that you have come with answers prepared and your responses would seem stiff and insincere. They want you to at least appear natural in interviews, and you have to exhibit a good amount of self confidence.

    You don't want to make it seem that you really need the job. On the contrary try to convince them that they need you, and that you're the only person who can accomplish the task.

  6. It's OK to take notes with you to refer to occasionally, but try not to read directly from a page as this will look like you are relying entirely on a text anyone could have written.

    Dress smartly.  If possible, wear a suit.  First impressions count so make the best impression on your future employer.
