  1. That's a lot of information to write out... in fact it's a book's worth!

So I would suggest getting one of the following books:

    Emily Post's Wedding Etiquette

    Martha Stewart's Weddings

    The Knot's Ultimate Wedding Planner

Something New: Wedding Etiquette for Rule Breakers...

    The Everything Wedding Book

    Hope this helps!

  2. other then that if you want a church wedding you should talk to the priest there to hear what he have to say (may demand pre marriage counceling, and have a thing or two to say in regards to ceremony) then nothing special

all up to you what you want to do, there is no law on what to do, its up to you what you want to do.

i know i discussed the topic with the bf, i am born european he is born in US, and due to how things apear likelyhood is unless we enlope then we'd be living in US when we got married, and such would have a US wedding

so yeah as far as from what i picked up, the only place you may meet hickups is with a church wedding, and then depending on the priest.
