  1. The next line below the 3rd row would put you at 20/70, but you will not be able to get contact lenses without a prescription from a qualified Optometrist/Ophthalmologist.  You cannot just go to the store and say I have 20/70 eye sight there is more to that when you get contact lenses.  They take the diameter of your eye and some other measurements and from there they are able to make the lens.  It usually has some negative number if you are near sighted or some positive number if you are far sighted.  As for glasses, they require a little less than the contact lenses, but will still require a SIGNED prescription to fill the order.  I'm not understanding why they want to recharge you for the exam rather than just have you pay a couple of bucks for a new prescription, unless they just don't keep the information on file.  How long has it been since the last exam?  Also, if it comes down to paying, the $60 is cheap, I pay a LOT more than that for my eye exams.  Maybe you could borrow the money or go where you want to purchase the glasses/contacts and see if they have some kind of discount.  Good luck!

  2. You should not have to pay for another test, simply ask them for the info. It should be in your file and that information is yours.

  3. They make no sense.  You paid for an eye exam and prescription.  You lost the prescription and list of sources where to buy your contacts.  They should be able to provide you with a copy.  Report this office to the next level of management and explain the situation.  Do not take the unprofessional response you received from this office as gospel.  Obviously, all they care about is your money not your eye care!  Report them.
